Aneurysm of the Right Hepatic Artery whith was Perforated to the Common Bile Duct

Abstract
A ease of aneurysm of the right hepatic artery, which was perforated to the common bile duct and resulted to severe hemobilia, is herein reported and the literatures has been reviewed.
Among the various causes of hemobilia, aneurysm of the hepatic artery constitute the most rare one of causes.
A 53 years old male patient was admitted to the department of surgery on Nov, 16. 1968, because of marked right upper quadrant pain, hematemesis and melena for one day duration. He had intermitent RUQ pain and tendernes which was radiated to the bact and right shoulder during last year. On phy-sical examination, markedly distended gall bladder was palpable on RUQ area. About 1,000 ml of blood mixed bile was aspirated by multiple needle aspirations of the gallbladder preoperatively.
Under the diagnosis of hemobilia, operation was performed and showed that a hen¢¥s egg sized true aneurysm of right hepatic artery was perforated to the common duct and the gallbladder was full of old bleed clot mixed with fresh blood about 300 ml in amounts, without any gall stone or paracites. The common duct showed moderate dilatation, 1.5 cm in diameter and contains old blood clot.
After resection of aneurysm with ligation of right hepatic artery and cholecystectorny with T-tube drainage of the common duct, patient was recovered well wightout any complication. Post operativeT-tube cholangiogram and liver function test showed no abnormality.
The patient was discharged on 28th hospital day uneventfully.
